Our Services
S&N Home Care is approved by the State of Indiana Medicaid program to provide Home Care services. We also service self-pay clients as well as private insurance pay clients. Our Services include:

Attendant Care Services:
Attendant care services provide direct, hands-on care to clients for their daily functional needs such as personal care, assistance with mobility, meal preparation, toileting, etc.
Home and Community Assistance/Homemaker Services:
These services provide instrumental activities of daily living for participants in their homes. Services may include light housekeeping, laundry, meal preparation, and errands.

Structured Family Care Giving:
Structured family caregiving is an arrangement in which a participant lives with a principal caregiver who provides daily care and support to the participant based on their daily care needs. Caregivers receive training on the participant's assessed needs from a paid agency and are paid per diem stipends for the care and support they provide.

Other Services we will provide in the future include home-delivered meals and Transportation Services.
